A Community Health Worker is here to help.
A Community Health Worker (CHW) is a trusted member of your community who understands your needs and will link you to health and social services. CHWs walk alongside clients to set goals and assist in achieving them. A CHW will:
- Meet with you at your convenience
- Help you set goals
- Guide you through the health care system
-
Connect you to the right medical care for your needs — including primary care, dental, mental health, specialty care, and substance use treatment
- Help you manage your health conditions and prescriptions
- Help you reduce hospital and emergency room visits
- Link you to local services and resources — such as food, clothing, housing, financial help, transportation, education, and employment
- And much more
